You can find your customer reference on the email we sent when you joined Dojo and it begins with SP. You'll also find it in your Dojo app.
Please confirm the details of your previous card payments provider and card machine lease provider (if applicable), and attach final VAT statements showing any cancellation fees.
Please enter the cancellation costs you're claiming, and attach proof of payment to your previous provider(s). This can be a bank statement or receipt, clearly showing the cancellation fee paid.
